Description:
Conducts appropriate evaluations, assesses home environment (as appropriate), and identifies equipment needs relative to speech/language function.
Identifies functional speech defects and establishes a Plan of Care (POC) to improve patient's function.
Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.
Recommends communication devices/aids as indicated.
Confers with referring physician and other Kindred at Home clinical staff to ensure coordinated and comprehensive care contributes to the total Plan of Care (POC).
Documents the course of patient care including progress made and continuing need for treatment.
Provide patient and family/caregiver education on tasks and resources that will assist with continued functional independence.
Identifies patient and family/caregiver needs for other services and refers as appropriate.
May supervise home health aide in procedures directly related to the speech-language therapy Plan of Care (POC).
As appropriate, participates in case conferences, in-services and other meetings required to ensure coordinated and comprehensive care.
Performs other related duties as assigned or requested.
Skills
Must possess current Certificate of Clinical Competence (CCC) in Speech-Language Pathology from the American Speech-Language-Hearing Association (ASHA) or be CCC eligible and hold current state license as applicable.
Proof of current CPR certification.
Current driver's license.
Must have reliable transportation and insurance.
Education
Graduate of a master's level program in speech-language pathology which is either accredited by the Educational Standards Board or whose course work and practice meet ASHA certification requirements •
