Case Manager

• Responsible for telephonically (remotely) assessing, planning, implementing, and coordinating all case management activities with patients to evaluate the medical needs of the member to facilitate the member’s overall wellness.
• Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration.
• Utilize assessment skills and discretionary judgment to develop plan of care based upon assessment with specific objectives, goals and interventions designed to meet member's needs and promote desired outcomes.
• Coordinate services between Primary Care Physician (PCP), specialists, medical providers, and non-medical staff as necessary to meet the complete medical needs.
• Provide patient and provider education.

• Holds a Compact Registered Nurse License and should be residing within Florida, Indiana, Missouri, or Tennessee.
• 3-5 years of hospital Medical/surgical experience
• Associate degree or nursing diploma required.
• Requested criteria above average computer skills (Microsoft Office Suite).

• Triage, case management and/or rehabilitation experience preferred.
