Job Summary: We are seeking a compassionate and experienced Family Nurse Practitioner (FNP) to join our healthcare team. The ideal candidate will be committed to providing holistic and patient-centered care, with a focus on preventive services, chronic disease management, and health promotion. As an FNP at our community health center, you will work collaboratively with our medical and support staff to deliver high-quality care to our diverse patient population.
Key Responsibilities
Patient Care:
- Conduct comprehensive assessments of patients, including physical examinations and medical histories.
- Diagnose and manage acute and chronic health conditions.
- Develop and implement individualized treatment plans in collaboration with patients and their families.
- Order and interpret diagnostic tests and procedures, including laboratory tests and imaging studies.
- Prescribe medications and other treatments as needed.
- Provide patient education on health management, disease prevention, and wellness strategies.
- Perform minor procedures and interventions as required.
Clinical Operations:
- Maintain accurate and up-to-date patient records in the electronic health record (EHR) system.
- Collaborate with other healthcare providers, including physicians, nurses, and specialists, to ensure coordinated care.
- Participate in case reviews, quality improvement initiatives, and clinical meetings.
- Provide guidance and mentorship to nursing and support staff as needed.
Community Engagement:
- Engage with community organizations and participate in outreach programs to promote health education and preventive care.
- Address social determinants of health and work to reduce barriers to accessing healthcare services.
- Advocate for patients and support efforts to improve health equity in the community.
Qualifications:
- Master’s degree in nursing with a focus on Family Nurse Practitioner (FNP) from an accredited program.
- Current and valid certification as a Family Nurse Practitioner (FNP) from a recognized certifying body.
- Active, unrestricted RN license and FNP certification in [State].
- Minimum of [X] years of clinical experience in a family practice or primary care setting preferred.
- Strong knowledge of evidence-based practice guidelines and healthcare regulations.
