Country Manor Home Health Care has been proudly serving central MN for over 50 years. We care for people of all ages in various settings, specializing in numerous areas that are locally and nationally recognized. Our Home Care Team is highly respected among their peers and works alongside a skilled clinical team as a cohesive group of experts.
We are in search of a Client Services Coordinator, who will be pivotal in managing and coordinating all aspects of in-home care services. This role ensures that clients receive exceptional care by liaising with caregivers, healthcare professionals, and referral sources, while building strong relationships with both existing and potential partners. Acting as the first point of contact, the Client Services Coordinator is responsible for operational management, ensuring the quality of client care, and fostering relationships that promote the growth of the Home Health Agency.
As a Client Services Coordinator, you will:
Coordinate and oversee the scheduling of in-home care services, ensuring caregiver assignments are efficiently managed to meet client needs.
Maintain clear communication with clients, families, caregivers, and healthcare professionals to ensure seamless care delivery.
Monitor the quality of services, ensuring they meet the highest standards, and adjust plans as needed to improve outcomes.
Support office operations through administrative tasks such as managing referrals, tracking schedules, payroll, and other office coordination duties.
Handle and resolve any schedule or staffing changes, providing timely updates to clients, caregivers, and internal management.
Build and nurture relationships with healthcare providers, clients, community representatives, and referral sources, ensuring they are comfortable recommending our agency.
Actively participate in community activities including but not limited to industry-related committees, boards, community projects, fundraising, etc. to represent Country Manor Home Health Agency, initiate and strengthen referral relationships
Engage in outreach and networking efforts to develop new referral sources from various healthcare organizations, such as assisted living facilities, hospitals, and skilled nursing facilities.
Maintain a detailed database of prospects, clients, care providers, and referral sources for relationship management and agency growth.
Uphold a positive, professional demeanor while managing multiple priorities in a fast-paced office environment.
Ensure all communications and documentation are accurate, timely, and adhere to agency protocols.
Qualifications:
Excellent communication and organizational skills, with the ability to build and maintain professional relationships.
Previous experience in healthcare or home health services is preferred.
Ability to multitask and manage operational demands effectively.
Compassionate and patient demeanor, ensuring the best possible experience for clients and their families.
