Registered Nurse (RN) - Home Health

Sierra Home Health & Hospice is currently seeking an experienced, qualified, and compassionate Registered Nurse (RN) to care for our patients in their home environment. The ideal candidate will have excellent interpersonal skills to assist with physician communications as well as the ability to educate patients and family members. With locations in Las Cruces & Truth or Consequences, Sierra has been serving southern New Mexico for over 20 years.

Duties & Responsibilities:

• Evaluate and treat patients using the most current technology and practices
• Develop initial plan of care based on communication with the physician and patient assessment
• Participate in clinical outcomes monitoring, follow up, and agency performance improvement initiatives
• Manage and educate Home Health Aides and LPNs performance in implementing nursing services
• Collaborate with an interdisciplinary team to provide highest level of care

Requirements

Qualifications & Requirements:

• Current and unencumbered license to practice as a Registered Nurse in the state of New Mexico
• Minimum of one (1) year of home health nursing experience
• Current CPR certification
• Reliable transportation, valid driver's license with no DUI's, revocations, or suspensions in the last 5 years
• Exceptional customer care
• Excellent written and verbal communication skills
• MS Office skills (Word, Excel, PowerPoint
