The Adolescent D&A Service Coordinator provides a full range of case management services for clients diagnosed with drug or alcohol needs.  This position has a responsibility for partnering with the client and/or their parent/guardian to identify resources that will build upon the client’s strengths and support and maintain the client’s path towards recovery.   This person ensures that these clients and their families have access to services on a continuum of care by identifying and referring to appropriate services. Major responsibilities include assessment of client’s needs, determining level of care, developing client service plans, and facilitating the client’s treatment to address their problems relative to drug or alcohol usage when warranted. Must develop a rapport with client serving as their advocate, assisting throughout the provisions in their services plan, and monitoring their progress. This employee will also perform the work in accordance with established regulations, policies, procedures, and supervisory review.

DUTIES AND RESPONSIBILITIES
 
  1. Conducts interviews utilizing the appropriate assessment practices to obtain the necessary information from the client, families, and/or involved collateral for the identification of services needed.
    1. Determines appropriate level of care based on current placement guidelines.
  2. Determines appropriate funding arrangements and authorizes services for clients with drug or alcohol needs.
  3. Orients and guides clients through the program, policy, procedures, and treatment process:
    1. Be the “go-to” resource for the client served
    2. Update the client’s service plan as needed
    3. Coordinates and provides access to available alcohol and other drug or ancillary services, such as education, medical, and transportation.
    4. Prepare for and facilitate service planning meetings as well as provide follow-up after meetings
    5. Monitors client treatment progress
    6. Assures that there are effective “safety net’ resources for the client served
    7. Clearly communicates to the client and their parent/guardian what they can expect from the system and what the system will expect of them
    8. Assure there is cross system coordination of services and that services are being provided
    9. Provides culturally competent services with consideration for race, religion, sexual orientation, gender identity, age and ethnic background
    10. Assist the client in developing and utilizing natural supports
    11. Provide a consistent positive outlook that encourages recovery and full inclusion in the community
 
  1. Prepares written reports and maintains client records.
  2. Utilizes and promotes recovery principles in an effort to promote client’s resiliency and recovery.
  3. Maintain monthly utilization reflecting daily activity and submits documentation within constraints of policies.
  4. Maintains monthly contact with all clients and providers/system partners.
  5. Attend and participate in all appropriate meetings with system partners.
  6. Complies with and adheres to HIPAA and Drug and Alcohol confidentiality policies.
  7. Coordinates with families, providers, Children Youth and Families, Juvenile Probation, educational systems and any other community partners as necessary to contribute to client’s success.  Attends all ancillary meetings with systems involved (court hearings, permanency planning, IEP etc.).
  8. Provide educational advocacy by attending any school meetings and providing technical assistance as needed.
  9. Attends and participates in professional development and remains competent and knowledgeable of current trends, initiatives, resources, and services available for clients and families.
  10. Monitoring of service delivery.
  11. Serve as an advocate for clients.
  12. Perform other duties as assigned.

SKILLS AND KNOWLEDGE
  1. Preferred knowledge, training and experience in social casework principles and methods.
  2. Knowledge of addiction as a disease and its treatment.
  3. Knowledge of current social, economic, and health problems and available resources.
  4. Ability to provide the client with the correct treatment facilities and human services agencies.
  5. Ability to interpret regulations, policies, and procedures and apply them accordingly
  6. Ability to work with people from a variety of diverse agencies, backgrounds, resources, and communities.
  7. Good communication skills.
  8. Ability to work in a team-oriented atmosphere.
  9. Ability to plan and organize records, reports, and maintain multiple caseloads in an effective and timely manner.

ESSENTIAL REQUIREMENTS
 
  1. Possess Act 33, 34 and FBI Clearances.
  2. Possess or is willing to pursue approved credentialing through PCB.
  3. Have a valid Pennsylvania motor vehicle license, access to a personal vehicle, and have minimum auto insurance on an employee driven vehicle.
  4. Ability to work independently.
  5. Full time position requiring 38.75 hours per week.

EDUCATION AND EXPERIENCE
Bachelor’s Degree in Chemical Dependency, Sociology, Social Welfare, Psychology, Nursing, Criminal Justice or a related field and one-year direct service experience performing counseling or case management work in alcohol or other drug treatment setting. Or, any equivalent combination of experience and training.
