Sigma Healthcare Team is looking for experienced Home Health RNs to work with our client on one of their teams.

Shift: 8.00hr - AM

Summary:
Responsible for traveling to patient's homes or skilled nursing facilities in a localized area around the regional office to assess and perform skilled nursing services. As well as document assessments, actions and outcomes.

Responsibilities:

Evaluate patients and develop appropriate care plans
Apply care plans according to individual patients
Assess outcomes and manage care plan modifications as needed (documentation through OASIS software)
Provide nursing services based on the care plan devised (G-tubes, J-tubes, Trachs, wound care, etc.)
Supervise nursing assistants in their everyday work such as bathing and grooming
Administer medication as instructed
Manage emergency procedures

Requirements:

Acute care experience
At least 2 year of experience in Home Care as an RN
Currently BLS certified
Valid driver's license
OASIS trained
Preferred EPIC experience
