Aliados Health (AH) is a network comprised of 17 community health center members with sites in Marin, Napa, Sonoma, Contra Costa, Solano, and Yolo counties. Formed in 1994, our mission is to improve access to, and the quality of care provided for underserved and uninsured people in our six counties. Aliados Health’s major focus is to strengthen the capacity of member health centers to act as a regional network of comprehensive primary care that provides access to consistently high-quality clinical care, effective care coordination and reduce health disparities.
Additionally, Aliados Health serves as a HRSA designated Health Center Controlled Network, which serves an expanded region in Northern California across a thirteen-county region: Marin, Mendocino, napa, Nevada, Placer, Santa Clara, San Francisco, Shasta, Sierra, Siskiyou, Solano, Sonoma, and Yolo through projects in Health Information Technology, Analytics and Health Equity.
Reporting to the ASE Program Manager, the Program Coordinator II has responsibility for coordinating monthly provider check-ins, completing reporting, Treatment Authorization Requests (TAR) and invoicing requirements related to the Aliados Health CalAIM Administrative Services Entity (ASE). The ASE provides administrative and clinical support services such as claims submission, reporting, care plan review and programmatic tracking for health centers and other community-based organizations in our 6-county region. The Program Coordinator II will be part of a small team and will be responsible for coordinating and ensuring that ASE reporting, tracking and invoices are done in a timely matter. The Program Coordinator II will coordinate and assist in scheduling monthly provider check ins and trainings related to ASE activities. This position also helps support onboarding of new clients, tracking program revenue, and other related activities.
· Work with the Community Transformation Team to support data collection, data entry, data analysis, and maintenance of high-quality data to support the Administrative Services Entity (ASE).
· Coordinate and schedule monthly provider check in meetings
· Provide prompt correspondence via email or secure file transfer to let providers know status of TARs, reports and invoices.
· Help receive appropriate information from participating clients, follow up on missing data elements, and enter data in appropriate systems, including health plan portals.
· Maintain all documentation per ASE protocols in appropriate and protected files and folders.
· Ensure that Protected Health Information (PHI) is appropriately stored and protected in compliance with Aliados Health policy and HIPAA/related regulatory requirements.
· Submit Treatment Authorization Requests, Release of Information, clinical quality data, claims, Member Tracking Files, and other appropriate reports needed monthly for each enrolled client entity and member enrolled.
· Communicates in a professional and timely manner with Aliados Health, member health centers and key partners.
· Participate in assigned team.
· Document workflows and other processes as needed for program sustainability and growth.
• Work with staff from member health centers and partner organizations.
• Oversee or guide trainings and other sessions for the ASE as required.
• Assist others in department and organization as requested.
• Participate in organization-wide meetings, committees and other activities as requested.
· Support value-based care transformation efforts as requested and/or facilitate collaboration between Informatics, QI, and Population Health.
• Two years of Program Coordinator I or equivalent work experience.
• Minimum one year experience in health care, public health, social service, or related areas.
• Minimum one year experience using spreadsheets.
• Moderate computer proficiency in the areas of spreadsheets, graphics, database, and word- processing software (MS Office products) in a Windows environment required.
• Associate degree or equivalent experience.
• Experience within a health care organization or safety net non-profit in an administrative or program support role preferred.
• Experience using Electronic Health Records (EHRs)
• Experience using Medi-Cal provider portals
• Ability to manage multiple virtual meeting technologies (Webex, Zoom, Teams, other).
• Strong organizational skills with ability to prioritize projects and meet deadlines.
• Computer literacy and intermediate to advanced ability to use required software applications, especially Microsoft Office 365 and related online applications.
• Exercises good judgment, problem solving and decision-making skills, ability to identify needs and suggest solutions.
• Strong written and oral communication and strong interpersonal and customer service skills, friendly personality, and comfortable speaking to a group.

Physical Requirements: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to sit; use hands to handle or feel; and talk and hear. This job is rated for Light work, requiring the ability to lift or move up to 20 lbs. on occasion. Close visual acuity to perform activities such as reading and viewing a computer terminal are required. This position requires occasional travel throughout our member counties and may require the use of a personal vehicle. Ability to travel as required for meetings or conferences, automobile and valid driver’s license preferred or the ability to get to various work sites.
