PRIMARY RESPONSIBILITIES:
• Case Manager will develop and review service plans, and implement activities, which ensure access to the needed services documented in the service plan.
• Case Manager will advocate for the client at meetings, i.e. multi-disciplinary, utilization review, school staffing, court and permanency staffing’s, etc.
• Case Manager will monitor the quality of care of all clients on caseload. Monitoring will include home and school visits and clinical services.
• Preparing and maintaining case record documentation to include case management notes, reports, etc.
• Case Manager is responsible for scheduling all appointments, including contacting responsible parties to coordinate appointments, giving directions as needed or arranging transportation and confirming all appointments 24-hours in advance.
• Case Managers will need to monitor attendance and follow-up with letters/telephone calls if client fails to keep an appointment.
• Case Manager shall assume responsibility for coordinating and scheduling all psychiatric evaluations, psychological assessments and ensuring that all client appointments are confirmed in advance with the guardian/caregiver.
• Case Manager will perform case management duties as needed for new and existing clients such as contact child welfare case managers, securing previous evaluations, and case plans, etc., coordinating transportation, documenting all case management activity in progress notes.
• Case Manager is responsible for maintaining a caseload of 20 clients and producing a consistent volume of quality and acceptable work.
• Case Manager will perform Quality Assurance Reviews for all clients on caseload, to ensure that all clients have individualized treatment plans, service plans, etc., and that all documentation and services address specific goals and objectives as ordered by the treatment team.
• Work as a member of the multidisciplinary team, with mutual respect, to ensure well-defined rights, responsibilities, and resources.
