Job details:-

The MDS Coordinator coordinates the completion and transmission of Minimum Data Set and Care Plan with interdisciplinary clinical team.

Job responsibilities:-

Coordinates the scheduled completion date for Minimum Data Set and Care Plans with interdisciplinary clinical team.
Completes and transmits Minimum Data Set to Center Medicare & Medicaid Services in accordance with regulatory guidelines.
Completes individualized care plans for each resident.
Participates in resident care plan meetings.
Performs chart audits to ensure clinical documents meet regulatory guidelines.
Prepares and reports clinical information for PPS and Utilization Review meetings with interdisciplinary team.
Prepares Medicare certification and re-certification for physician signature.
Conducts staff in-services related to Minimum Data Set guidelines and RAI manual.

Qualifications:-

Associate degree is required.
5 years of clinical and utilization experience in a Long-Term Care or skilled facility is required.
Current Georgia License as a Practical Nurse (LPN) is required.
Resident Assessment Coordinator Certification is required.
Must possess a strong working knowledge of Medicare, Medicaid reimbursement guidelines, PPS and Rugs categories as it relates to the MDS process.
