Responsibilities and Duties
- Provides Functional Support Services (FSS) to eligible children and their families. Services are delivered in the home or community settings. A Family Outreach Specialist is responsible for a wide variety of activities. He/she must be eclectic in nature and able to concentrate on identifying and addressing the needs of the families who receive services and develop and maintain a positive, supportive relationship with both the family’s referral sources and community supports. She/he needs to be competent in behavior management strategies and parent support functions and be able to provide psycho-education to families.
- Targeted Case Management (TCM) services are provided to CAFS clients who would benefit from assistance in order to access medical, social, educational and other needed services in the community.
- Should be able to make an accurate needs assessment and take steps in order to have these needs met. These needs are often diverse and will vary from one family to the next.
- Be aware of and assist in working with the child's natural supports, and assist in expanding these supports as deemed necessary by the child's family and other caregivers.
- The Family Outreach Specialist is a part of the child's treatment team and is responsible for relaying information through the appropriate channels. This may include attendance at assessment team meetings or consults with other providers as needed.
- Required to attend supervision and in-services on a regular basis.
- Must be aware of and adhere to the SMHC Policy and Procedure manual.
- Will be aware of and adhere to state and federal mandates regarding service provisions.
- The Family Outreach Specialist is required to keep regular documentation of all activities.
- The Family Outreach Specialist will meet the clinical performance standards as developed by the agency.
- The Family Outreach Specialist will need to work a flexible, nontraditional schedule to ensure the continuity of care and optimal support services.
- The ideal applicant must possess a Bachelor's degree in Social Work, Psychology, Human Services or the equivalent and preferably have one year of experience working in the field.
