STATUS: Full-time
SALARY: $55,000 - $57,000 Annually

OVERVIEW: The Love146 Survivor Care Transitional Services Specialist is a full-time position. The Survivor Care Transitional Services Specialist supports participants ages 16 and above in the Connecticut Survivor Care Program to achieve their goals as they prepare to transition into adulthood. Transitional Services programming focuses on helping youth with secondary and post-secondary achievement, employment attainment, housing, and other related case management services

JOB RESPONSIBILITIES
Direct Services
- Implement Love146’s Survivor Care program’s Transitional Services programming services with a standard of excellence.
- Help participants to identify individual Care Plan goals as well as measurable objectives for obtaining those goals.
- Provide direct services to program participants to help them achieve their Care Plan goals. Services may include, but are not limited to the following: (1) resume building, (2) soft skills and mock interviews, (3) job searches and placement assistance, (4) research on post-secondary educational opportunities, (5) post-secondary financial aid, and scholarship application assistance, (6) post-placement/enrollment check-in/assistance, (7) emergency, transitional, and short-term housing assistance, and (8) case management to address related issues such as gaps in basic needs, transportation, and childcare.
- Liaise between program participants, educational institutions, employers, housing resources, childcare resources, and other government and community-based agencies to support program participants in achieving their Care Plan goals.
- Track participant’s progress in obtaining their Care Plan goals and modify services and goals as needed.
Service Coordination and Implementation:
- Identify, build, and maintain relationships with existing post-secondary education institutions, vocational service providers, employers, housing programs, landlords, and other related providers to increase access to comprehensive services and support for participant success
- Serve as a resource and provide education to educational institutions, employers, landlords, and other providers so that they support this population.
- Participate in training and outreach to community partners to increase awareness of Survivor Care services and Human Trafficking as needed.
- Document and track appropriate data, enter and update participant data, and support program monitoring and evaluation.
- Consult and collaborate with internal Love146 departments and programs.
- Collaborate and participate in Love146 meetings.
- Help promote a culture that fosters a passion for the mission, cooperation, open and frequent communication, teamwork, and a common vision.
- Performs other activities as assigned by the Director of US Survivor Care.
QUALIFICATIONS & EXPERIENCE
- Bachelor in a related field, such as human resources, social work, business, or education preferred; or Associate in a related field + 2-3 years of related work experience.
- Prior transitional services (e.g., workforce development, job placement, post-secondary education, and/or housing placement) experience.
- Experience working directly with adolescents and young adults, preferably with high-risk populations.
- Experience providing community-based services preferred.
- Valid driver’s license, a clean driving record.
