Minimum Qualifications:

One (1) year of professional experience in developing, implementing or evaluating human services, sociology, public health, health care services, or health care insurance programs; and a bachelor’s degree in Human Services or Education Field; or Any equivalent combination of experience and training that includes at least one (1) year of professional experience in developing, implementing or evaluating human services, education, sociology, public health, health care services, or health care insurance programs.

Intermediate Computer skills to include, but not limited, to Microsoft Office Products.

Role Description:

Job Duties and Responsibilities:

Strong organizational skills with demonstrated ability to work independently under general supervision

Build, develop, and grow business relationships vital to the success of the PA Link in the assigned region

Monitor program objectives through review, evaluation, and analysis of program policy, guidelines and budgetary data/information. This includes but not limited to the following:

  • Provide guidance to Service Area Oversight Committees and Lead Coordinators within the region in order to enhance Service Area performance and maintain compliance with program policy.
  • Take a lead role in network collaboration and partner development activities.
  • Provide guidance to Service Areas on programing related activities
  • Create and Monitor Lead Coordinator developmental plans and activities.

Assist with Person-Centered Counseling (PCC) activities within the region. PCC represents a critical service of the ADRC whereby the goal is to facilitate informed decision making about long-term services and supports (LTSS). Activities include:

  • Organize and conduct PCC training to ensure the service is offered in all counties within the region.
  • Identify on-going training/educational needs for PCC Counselors and organize activities to address those needs within the region.
  • Ensure reimbursement standards for PCC are being met by reviewing and validating PCC consumer documentation in the Social Assistance Management System (SAMS).

Manage day-to-day operational aspects by:

  • Effectively communicate program expectations and progress to the Program Manager in a timely and clear fashion.
  • Where required, confer with other department staff for the acquisition of information necessary for partnership building and referrals.
  • Identify and resolve issues and conflicts within the service areas of the region.
  • Track performance indicators and report outcomes for the service areas within the region.
  • Coordinate internal and external meetings (face-to-face and conference calls).
  • Create and give presentations and provide verbal and written updates on project status to a variety of audience levels.
  • Assist with development, standardization, implementation, and maintenance of PA Link policies and procedures as well as current or potential grant opportunities.
  • Monitor program objectives through review, evaluation, and analysis of program and budgetary data.
  • Respond to inquiries/questions related to the PA Link in designated region.
  • Analyze service area budgets and quarterly activity reports for compliance.
  • Other duties as assigned in relation to PA Link network/ADRC Program.
