Licensed Independent Social Worker - Clinical

Compensation: $70,000 - $75,000 Per Year Salary + Bonus Opportunities
Position Summary:
- As a Licensed Independent Social Worker (LISW), you will conduct comprehensive psychosocial assessments of geriatric residents in nursing homes and assisted living facilities to identify their unique needs, preferences, and psychosocial challenges
- Collaborate closely with residents, their families, and interdisciplinary care teams to develop individualized care plans that address social, emotional, and mental health concerns effectively
- The Licensed Independent Social worker will provide evidence-based therapeutic interventions, counseling, and support services to residents facing various mental health issues, including depression, anxiety, grief, and adjustment disorders
- Advocate for the rights and dignity of geriatric residents, ensuring access to appropriate community resources, social services, and supportive care options
- Foster a supportive and inclusive environment within long-term care facilities, promoting resident engagement, socialization, and emotional well-being
- Document assessment findings, treatment plans, and interventions accurately and timely in accordance with regulatory standards and organizational policies
- Collaborate effectively with facility staff, including nurses, physicians, and administrators, to coordinate long term care plans, address resident needs, and optimize treatment outcomes

Requirements:
- Licensed Independent Social Worker in the state in which you applied (OH, PA, WV, VA, IN)
- Past counseling experience - Long Term Care experience preferred
- Create a climate of patient-centered care to include confidentiality, empathy, respect, and collaboration
- Develop individualized Treatment Plans based on information collected during initial screenings/ evaluations and the ongoing assessment of needs, strengths, preferences, and goals
- A strong work ethic, attention to detail, a willingness to learn, and excellent empathy
- Familiarity with the geriatric population and long-care settings is a plus
- Candidates must have excellent skills in diagnostic assessment and counseling and desire to work in a fast-paced team environment
