Minimum Qualifications:

  • Graduate degree in Human Services (psychology, social work, or a related field), at least (5) years of clinical experience in working with children, youth, and young adults (ages 0-21) with complex needs, and a minimum of three (3) years of supervisory experience.
  • Ability to orally communicate effectively in English in all levels of group and individual contacts/setting.
  • Ability to communicate effectively in writing, using the English language, including computer-based communication using e-mail, word processing, and data based systems.
  • Knowledge of Positive Practices/Approaches and the Recovery Model frameworks in Pennsylvania.
  • Experience in working with multiple stakeholders across systems and facilitating collaborative practices/supports for youth with complex needs.
  • Intermediate computer skills including the use of Microsoft Office software and web-based applications.

Role Description:

The clinical consultant will support the planning activities for children, youth, and young adults with complex needs who are referred to DHS from a variety of levels of care and settings, some of which include inpatient psychiatric hospitals, residential treatment facilities, artificial community-based settings, family homes, and others. Support of these planning activities often entails providing clinical input and guidance, ensuring that best and promising practices are considered and explored from a regulatory and clinical standpoint, promoting trauma-informed approaches, encouraging strategies which enhance resilience and recovery. This will occur in the following ways:

Team Coordination and Oversight

  • Support the complex needs planning team with the director of the complex needs planning initiative.
  • Participate in regular check-ins with other clinical directors and the director of the complex needs planning initiative.
  • Interface and coordinate with other offices and systems as needed to support complex needs planning efforts.

Referral Support and Technical Assistance

  • Support referral intake/processing by monitoring the Department’s resource account, reviewing referrals, creating case conceptualizations to brief the DHS complex needs team, requesting additional information as needed, coordinating with regional and central office staff to join local planning efforts, and updating the tracking sheet with new referrals.
  • Participate in regular internal referral tracking calls, briefing the team on new referrals, updating the team on the status of assigned youth and supporting other team members in their efforts to brainstorm and identify solutions.
  • Support development of materials and tools for the complex case planning process, including providing training on their use to local entities as identified.
  • Support team efforts to complete biopsychosocial profiles, service grids, and transition plans for youth as requested – this includes developing local capacity to use these tools to meet future needs.
  • Support rapport building and networking among internal and external team members.
  • Participate in local level planning calls for youth referred to the Department, acting as a resource and providing TA as needs are identified.
  • Support transition planning efforts for youth moving to the community, with an emphasis on a proactive approach and long-term vision.
  • Assist Risk Management/Quality Management in efforts to support youth with their treatment and transition planning.
  • Provide recommendations based on record reviews as requested.
  • Refer to other clinical resources or subject matter experts as appropriate.
  • Advise local planning entities on best practices.

Policy and Capacity Building

  • Identify system challenges for the Department to consider.
  • Identify and provide training to the provider community, other professionals, and families as needs are identified.
  • Support, as requested, other components of the Department’s complex needs planning initiative, including the Pediatric Capacity Building Institute, the Blueprint for Youth with Complex Needs, Regional Collaboratives, etc.
  • Inform policy development based on best practices and trends.
  • Maintain current knowledge of community resources across systems (MH, ID/A, CYF), regionally and statewide to connect internal and external partners as needed.
  • Remain up to date on applicable research and best practices, maintaining files to share as needed.
  • Various tasks as assigned.
