TEMPORARY CARE COORDINATOR POSITIONS

These assignments are estimated to go from October 2023 through December 2023, and potentially extended into 2024. The lengths of the assignments are always dependent on business need and dates may change. While the assignments would be at the Alliance, if selected, you would be an employee of a temporary employment agency that we would connect you with. 

These Care Coordinator positions are within our Community Care Coordination Department. This team coordinates care and empower members to achieve their health and wellness goals through collaboration and by facilitating member relationships with community providers. We are filling multiple temporary assignments in the following areas, please let us know which assignments you are interested in as you complete your application.

  • Adult Complex Case Management - on this team you will assist assigned team with the coordination of health care activities and/or health education activities for Alliance members. You will help obtain and/or enter authorization requests for services and follow up to ensure and coordinate delivery, such as of hospital beds and complex durable medical equipment. You'll facilitate prior authorization of services with provider offices and community agencies and identify gaps in care for members and arranging appropriate and timely resolution with internal and external agencies. You'll work with a multi-disciplinary team with Care Coordinators, Social Workers and Registered Nurses to conduct telephone interviews with members, significant others and family members to determine if care needs are being met or if additional services are needed.
  • Pediatric Complex Case Management - this team is a collaborative, multi-disciplinary team with Care Coordinators, Social Workers and Registered Nurses, that support one another while case managing and advocating for children with complex medical and developmental issues. It is a busy team that always makes time to come together, constantly communicates, and delegates to remain efficient and productive. They are looking for another great team player, who also likes to have some fun in between the hard work.
  • Enhanced Care Management (ECM) and Community Supports (CS) - is a newer team and acts as liaisons between the ECM and CS Program providers and community agencies to promote effective implementation. On this team, you will assist the ECM team with providing health care coordination activities for Alliance members. You will establish and maintain effective working relationships with provider offices, County departments and other community agencies, and internal stakeholders related to care coordination for members, disease management, and health education. 


WHAT YOU'LL NEED TO BE SUCCESSFUL

  • Knowledge of:
    • The principles and practices of managed health care and health care systems
    • Customer service
    • Care coordination principles and practices is required for some assignments
    • Health education principles and practices is required for some assignments 
  • Ability to:
    • Understand and communicate the workings of the Alliance, particularly the Utilization Management and Complex Case Management, Community Care Coordination and Member Services departments
    • Competently navigate computer systems
    • Use Microsoft Word and Excel, and Alliance computer systems 
    • Work independently with minimal supervision 
    • Verbally communicate program mission, vision, and roles
    • Train and educate on health matters and/or make presentations to individuals and groups is required for some assignments
    • Effectively, clearly and independently document, summarize and resolve member's concerns and inquiries
  • Education and Experience:
    • High school diploma or equivalent and 
    • Three years (or an Associate's degree in health, social services or a related field or Medical Assistant certification and a minimum of two years) of relevant experience in a health care setting interacting with members, patients and/or providers in meeting service needs; or an equivalent combination of education and experience which would provide the required knowledge, skills and abilities may be qualifying

 

OTHER DETAILS

  • While this position is connected to one of our Alliance offices, we are in hybrid remote/in-office work environment right now and we anticipate that the interview process will take place remotely.
  • Our Alliance office locations have officially re-opened after the pandemic, and while some employees may work in full-time telecommute schedules, attendance at quarterly company-wide events or department meetings will be expected.
  • Based on the nature of work, this position may require onsite presence, which is dependent on business need. Details about this can be reviewed during the interview process
  • This is a temporary position, and does not provide the benefits that are listed below (it is standard language from our regular job posts and cannot be altered or removed)

 

Scotts Valley pay range
$25$31.55 USD

Additionally, all positions at the Alliance are required to meet these minimum qualifications.
