Care Coordinator

Job Profile:

What we need for this job:

• Bachelors in Psychology, Social Work, Counseling or related degree
• Collaborative, and client centered
• Organization is essential to managing client information, appointments and follow up items
• Collaborative team member who is ready to jump in, support our clients and team
• Strong community outreach experience and desires, ability to get to and from outreach engagements.

• Bachelor's Degree in Psychology, Social Work, Counseling or a related field OR one-year experience in the behavioral health field for non-related Bachelor's degrees
• Experience working with adults in an outpatient setting
• Must be 21 years old, have safe driving record, a current Colorado driver's license and ability to travel within Boulder and Broomfield Counties

Knowledge/Abilities:

• Working in a team approach that is collaborative and client centered
• Appetite and desire for learning
• Ready to gain experience, apply new learning, willingness to engage with our team
• Strong experience with technology
• Familiarity with insurance programs like Medicare and Medicaid
