Part Time Health Navigator Deployment Specialist - Syracuse, NY

The health navigator deployment specialist splits their time between on-site support in Syracuse, NY and remote monitoring of patients through the Biofourmis platform. While on-site the health navigator deployment specialist acts as the main liaison and support for the deployment and set-up of monitoring devices at patient homes. This includes, but is not limited to, scheduling/coordinating home visits, training patients, their caregivers and/or clinicians on how to apply and use equipment and related operational processes. They will work with members of the clinical care team to enhance better health and take the initiative to ensure clinical care and related supper services are made available to participants. While monitoring patients through the platform, the health navigator deployment specialist monitors alerts, and screen participants for medical or social concerns. The health navigator deployment specialist initiates audio-visual calls with participants who require remote patient monitoring in the home. They will report abnormal values or other participant concerns to the registered nurse per escalation protocols.

Responsibilities

  • Coordinates with clinical team on new patient referral and onboarding process
  • Assist with the installation of monitoring devices in skilled nursing facilities and patient homes
  • Assists with the troubleshooting of monitoring devices and escalation of technical issues to the customer success team
  • Educate patients, their caregivers and/or clinicians on device use and operating processes
  • Assist in the create and updating of operational procedures and work instructions
  • Monitor all alerts and escalate per protocols
  • Answer, direct, and follow up on incoming calls
  • Review patient compliance and maintains patient outreach schedule
  • Receive and perform patient calls and document all information obtained on the call in the appropriate charting systems
  • Support participants physical and emotional wellness with a holistic health approach
  • Addresses social determinants of health by supporting participants through loneliness, anxiety, and depression
  • Communicate concerns, changes in condition and all medical complaints to the Registered Nurse
  • Troubleshoot issues with technology and escalate technical difficulties that cannot be resolved
  • Other duties as assigned

Requirements

  • Must have a valid NYS Driver's License
  • Must be able to lift 30+ pounds repeatedly
  • Must be able to work remotely from a HIPAA compliant environment
  • Much have secure WIFI for remote work
  • Must available to work some evenings, holidays and weekends
  • High school diploma or GED required
  • Strong computer skills and ability to learn and use new technology
  • Empathy, compassion, and patience
  • Superior listening, verbal, and written communication skills
  • High level of professionalism and ability to work independently and with a team remotely
  • Strong problem-solving abilities with attention to detail
  • Ability to handle stressful situation appropriately
  • Adaptability, patience, and a positive attitude

Compensation will range from $18/hr - $21/hr depending upon experience.
