Physical Therapist (PT) – Home Health | Mission Home Health | Concord, CA
Mission Home Health is seeking a skilled and compassionate Physical Therapist (PT) to join our team in Concord, CA. This role offers the opportunity to make a meaningful impact by providing high-quality care to patients in their homes, helping them regain mobility and independence.
Position Details:
Location: Concord, CA
Compensation: Starting at $65/hour, DOE, open to negotiation
Employment Type: Full-time or Full-Time Light
Service Area: Concord and surrounding areas
Responsibilities:
- Evaluate and assess patients' physical conditions and develop individualized treatment plans
- Implement therapeutic exercises and techniques to improve mobility, strength, and coordination
- Educate patients and caregivers on exercises, safety precautions, and self-care strategies
- Collaborate with an interdisciplinary team to provide holistic, patient-centered care
- Maintain accurate and timely documentation of patient progress and treatment plans
Requirements:
- Active Physical Therapist license in the state of California
- Doctorate or Master’s degree in Physical Therapy
- Minimum of one (1) year of experience as a Physical Therapist (home health experience preferred)
- Current CPR certification
- Reliable transportation with valid driver’s license and auto insurance
