About the Position

The Clinical Care & Medication Management Registered Nurse (RN) is responsible for overseeing and coordinating the overall care of clients, ensuring their needs are met through effective communication and collaboration with caregivers, healthcare providers, and support services. This role focuses on promoting health, wellness, and safety through comprehensive care planning and monitoring.

In addition to care coordination, the RN provides support in managing clients' medication routines, ensuring safety, adherence, and understanding of prescribed medications. The role emphasizes education, safety monitoring, and medication organization, without direct medication administration. The RN collaborates closely with clients, caregivers, and other healthcare professionals to promote optimal health outcomes. 

Principal Duties and Responsibilities – Care Coordination:

  1. In conjunction with the Director of Member Services, develops a network of sub-contracted service providers. Participates in interviewing and selection of service providers, including home care agencies, transportation providers, Home Health Care, Hospice providers, etc. necessary to ensure exceptional delivery of services. 
  2. Obtains and reviews required documents from prospective service providers to ensure they meet Goodwin House at Home standards.
  3. Develops a network of vendor partners for referrals for non-covered services.  Maintains the network and manages the referral process.
  4. Schedules, conducts and documents pre-admission assessments of prospective members.  Serves on the Care Coordination Team providing input on decisions being made regarding applicants and members. 
  5. Conducts new member meetings.  Ensures that new members are welcomed into the program quickly and efficiently.
  6. Communicates regularly with members (minimum of one visit per year, quarterly phone calls and as needed). 
  7. Conducts annual and as-needed assessments of members to determine the need for services.
  8. Visits members in the hospital, in rehab centers or at home to determine the need for services and to provide reassurance.
  9. Manages the successful placement of appropriate services to address members’ needs.  Communicates with members regarding what Goodwin House at Home will pay for (what is ‘covered’), and members’ financial responsibilities. 
  10. Follows up with members to determine their satisfaction with services.
  11. In conjunction with the Care Coordination Team, manages any quality monitoring initiatives.  Serves on the Quality Monitoring Committee.
  12. Participates in informational seminars, including conducting presentations to small groups of prospective Members.
  13. Responsible for networking activities within the market area designed to enhance community awareness of the continuing care at home program.
  14. Utilizes AOD to document interactions with members, family members or other member associates, manage contacts, member follow-up, etc.
  15. Participates in the on-call schedule evenings and weekends. 
  16. Other tasks as assigned.

Duties and Responsibilities – Medication Management:

  1. Assess members/clients medication history and verify orders for accuracy.
  2. Promotes medication organization & management by filling member/client pill boxes and assist in organizing weekly or monthly medications to ensure proper dosing.  Identifying and addressing missed or incorrect doses to promote medication adherence and safety.
  3. Performs safety monitoring of members/clients for signs of medication non-compliance or confusion and notifying the appropriate healthcare providers or caregivers. 
  4. Ability to make informed decisions in urgent situations where medication management adjustments are needed.
  5. Educates members/clients on the importance of medication schedules and proper medication usage, and on medications, including side effects, interactions, and best practices for usage.
  6. Answer questions and provide guidance to members/clients and caregivers about their medication regimen.
  7. Assess the need for electronic pill dispensers or other technology solutions to enhance medication management.
  8. Coordinate the installation and training for the use of electronic medication systems when needed.

Job Requirements

  1. Understanding of older adults and the challenges of aging in place in one’s home in the community.
  2. Ability to establish and maintain positive relationships with customers, supervisors and peers.
  3. Experience serving older adults a plus, as well as providing services to individuals living in their own homes.
  4. Must be a graduate of an approved RN program.
  5. Must have current licensure in the Commonwealth of Virginia or compact state of residence.
  6. Must have current CPR certification.
  7. Possess a driver’s license, proof of automobile insurance and readily available, reliable transportation.
  8. Must be fully committed to the mission of Goodwin House Incorporated.
  9. Ability to work well under stress.
  10. Completes assignments accurately, works independently and manages time effectively.
  11. Must exhibit a high degree of initiative and creativity, good judgment, problem solving ability and professional ethics.
  12. Experience conducting assessments such as the Montreal Cognitive Assessment (MOCA), MMSE, Geriatric Depression Screen, Falls screen etc. preferred.
  13. Must be familiar with Windows operating systems, MS Office and email software.
  14. Must be willing to work remotely and/or share office space when necessary.
  15. Excellent verbal and written communication skills.
  16. A strong commitment to quality care and service and a sensitivity to the needs and concerns of older adults.
  17. Must be able to work flexible hours including rotating the after-hours on-call schedule and taking care of member’s needs nights and weekends, as required.
