About the Role
We’re seeking a Part-Time Home Health Physical Therapist to provide exceptional care to patients in the comfort of their homes. This role is under a Pay Per Visit model, with Starts of Care beginning at $150, and is open to negotiation.
Key Responsibilities:
- Deliver physical therapy assessments, services, and care plans tailored to each patient's needs.
- Assess patients’ home environments and functional status to promote safety and independence.
- Collaborate with an interdisciplinary team to ensure patient-centered care.
- Educate patients and families about exercises and activities to maintain progress.
- Maintain accurate and timely documentation of patient care and progress.
What You Bring:
- Valid Physical Therapist license in Oregon.
- At least 1 year of Physical Therapy experience, preferably in home health.
- Strong clinical, assessment, and communication skills.
- Reliable transportation to travel within the Bend area.
