Today, the Stillaguamish Tribe of Indians, a sovereign, federally recognized native tribe, is committed to serving our members, preserving our culture, and continuing the stewardship of our native lands.

 The Stillaguamish Tribe Wellness Clinic is open to Stillaguamish Tribal Members and their families, other enrolled Native Americans, and Tribal employees.

 

Position Summary

The Community Health Representative serves to enhance and protect the quality of life for Stillaguamish tribal members, through outreach to members in order to maximize the effective utilization of available health care, health education, disease prevention, dental, mental health and social services. The position operates under the direct supervision of the Office Manager. As a member of the health care team, they are also responsible for being a liaison between members and the community health care providers to ensure patients and families understand their conditions and treatments and are receiving appropriate care.   

 

Essential Duties and Responsibilities

  • Educating: Assists individuals and families in the timely, effective and appropriate use of health facilities, resources and preventative and curative practices and services available at the Stillaguamish tribal Wellness Clinic and other resources in the area and provide appropriate and informative materials.
  • Home/ Hospital Visits: Provides home visits for community members who are hospitalized, physically or mentally disabled or homebound, as directed by Tribal Wellness Clinic Staff and/or outside medical providers. Provide non-emergency medical monitoring in the home or at community events which could include vital signs, height, weight, and blood glucose.
  • Resource Access Assistance: Assists individuals interpreting eligibility requirements, completing necessary forms and applications and problem solving with various health and social services agencies, and providers. Provide advocacy and education services to members, facilitate access to needed health services that may be available and assist with scheduling appointments as needed.
  • Patient Treatment and Self-Care: Assists members and families with the proper use of medication and implementation of treatment and self-care plans as directed by Wellness Clinic and outside medical providers. Provides consistent ongoing medical contact with members who have chronic health problems and/or are confined to nursing or extended care facilities. Assists elders in the implementation of exercise, walking and stretching routines outlined by health care providers.
  • Medication distribution: Pick up and deliver prescriptions as needed to eligible community members, Provide limited courier services for medication delivery, in accordance with established policies and procedures (elders, disabled) under the guidance of Wellness Clinic Doctors and Nurse practitioners.
  • Meetings: Attends and participates in various community meetings and staff committees whose goals directly or indirectly relate to the Stillaguamish Wellness program objectives.
  • Health Fairs and Other Community Events: Coordinates and prepares information and topics for community health fairs and other types of tribal events focused on health education, exercise and other disease prevention activities including vaccine clinics and annual wellness checks.
  • Personal Contacts: Maintains regular contacts of a positive nature with elders, staff, and community members and as effective liaison with Indian Health Service, DSHS, local physicians and medical service providers, local fire department staff, and others to accomplish Stillaguamish Wellness Clinic objectives and preserve good public relations.
  • Reporting: Produces and distributes written and/or verbal reports to Wellness Health Clinic staff and outside providers in a timely matter as directed.
  • Other duties assigned by Office Manager.
  •  Assist elders with education and resources and provide assistance, signing up for Medicaid when age appropriate.
  • Assist members in locating providers for medical and dental services and setting up appointments as needed.
  • Assist members with applying for available health benefits and processing health bills. Work with H.R. Benefits Representative as needed.
  • Coordinate transportation through Stillaguamish Transportation department or other local resources within the local community to/from routine, non-emergency visits. Provide transportation to a patient without other means of transportation, only when necessary.
  • Attend specialty appointments with members when necessary to advocate.
  • Collaborate with daycare and youth service to provide health education to young children.
  • Support and nurture expectant new moms/ provide education.

 

Required Knowledge, Skills and Abilities

  • Knowledge of all confidentiality laws and regulations including HIPAA.
  • Knowledge and the ability to relate with the unique health needs of a Native American community.
  • Knowledge of basic medical processes and procedures.
  • Knowledge of medical resources and services available to respond to community needs.
  • Ability to safely lift, pull and push up to 50 pounds and safely and effectively transfer patients as required
  • Ability to communicate effectively with community members, visitors, and staff.
  • Ability to travel and be on call in order to carry out the responsibilities of this position.
  • Ability to demonstrate a passion for improving the welfare of others.

 

 Position Qualifications

  • High School Diploma or GED.
  • Must be a Certified Medical Assistant with one (1) year work experience in a health related field.
  • Must be willing to work flexible hours as may be necessitated by schedule medical/dental appointment and distant transports and community awareness events.
  • CPR/ First Aid Certification.
  • Must be able to complete CHR training course within 6 months of hire.
  • Must be able to pass a healthcare provider background check.
  • Valid Washington State Driver’s license and the ability to be approved by tribal insurance to drive company vehicles after completion of Defensive Driving course provided by the tribe.
  • Ability to pass background check and pre-employment drug screening.
  • Stillaguamish Tribe of Indians is a drug-free workplace. All employees are subject to a pre-employment drug screen.
  • All employees must pass a pre-employment FBI fingerprint background and state background check.  

Full Time 40 hours/week

Wage range:  $26.36 per hour
