Position Overview:
We are seeking a skilled and empathetic Social Worker to join our team. As a Social Worker at St. Gabriel’s Hospice, you will play a vital role in enhancing the quality of life for our patients and their loved ones. You’ll provide emotional support, counseling, and practical assistance to patients and families facing serious illness or end-of-life care.
Responsibilities:- Conduct comprehensive psychosocial assessments to understand patients’ needs, preferences, and goals.
- Collaborate with interdisciplinary teams to develop personalized care plans.
- Provide emotional support, counseling, and crisis intervention to patients and families.
- Assist with advance care planning, including discussions about end-of-life wishes.
- Connect patients and families with community resources, financial assistance, and support groups.
- Advocate for patients’ rights and ensure their voices are heard.
- Maintain accurate and confidential records.
Job Qualifications:- Education: Master’s degree in Social Work (MSW) from an accredited program preferred.
- Licensure: Licensed as required by the state where practicing. Current drivers’ license.
- Experience: One year of experience in healthcare, hospice, or related settings preferred.
- Skills: Working knowledge of community resources. Strong communication, empathy, and active listening skills. Knowledge of end-of-life care, grief, and bereavement support. Ability to work collaboratively in a multidisciplinary team.
- Transportation: Reliable transportation and valid auto liability insurance.
