Prescribing Pharmacist (In-clinic)
Job Summary: We are seeking a highly skilled and qualified Prescribing Pharmacist to join our team. The Prescribing Pharmacist will have the authority to prescribe medications under specific conditions and in accordance with applicable regulations. This role involves conducting patient assessments, diagnosing conditions, prescribing medications, and providing comprehensive medication management. The Prescribing Pharmacist will collaborate with healthcare providers, educate patients on medication usage, and ensure safe and effective treatment plans.
Location: Richmond, BC
Status: Contractor
Availability: Minimum 20 hrs/week
Responsibilities:
- Conduct comprehensive patient assessments, including reviewing medical history, conducting physical examinations, and interpreting laboratory results.
- Diagnose certain conditions based on assessment findings and determine appropriate medication therapy.
- Prescribe medications and adjust treatment plans as necessary to optimize patient care.
- Monitor patients' responses to medications, assess potential drug interactions, and ensure therapy is safe and effective.
- Collaborate with healthcare providers, including physicians, nurses, and other pharmacists, to ensure coordinated and comprehensive patient care.
- Provide patient education on medication usage, including proper administration, potential side effects, and precautions.
- Offer counseling and support to patients, addressing their concerns and questions related to medication therapy.
- Stay updated on the latest developments in pharmacotherapy and maintain knowledge of current guidelines and regulations.
- Maintain accurate and complete patient records, documenting assessments, diagnoses, treatment plans, and medication orders.
- Adhere to professional and ethical standards, ensuring compliance with applicable laws and regulations.
Qualification:
- Bachelor's or Doctor of Pharmacy (Pharm.D.) degree from an accredited pharmacy program.
- Licensure as a pharmacist in British Columbia.
- Strong knowledge of pharmacology, therapeutics, and medication management.
- Excellent clinical assessment skills and ability to interpret medical information.
- Effective communication and interpersonal skills to collaborate with healthcare providers and educate patients.
- Attention to detail and accuracy in prescribing medications and documenting patient information.
- Ability to work autonomously and make sound decisions in prescribing medications.
- Commitment to ongoing professional development and staying updated on best practices.
