The Program Coordinator will advance the care of the specific program patients throughout the organization. The role leads performance improvement initiatives through application of clinical expertise, knowledge of both clinical and administrative settings and frequent interactions with a variety of clinical and non-clinical roles. The program coordinator conducts comprehensive studies to evaluate activities and outcomes, manages policies and protocols, and reviews current best practice through networking and ongoing education. The coordinator may recommend, develop and coordinate improvement activities. The program coordinator may also be responsible for the development of educational materials, following-up with patients during hospitalization and partnering with various agencies and partners to reduce readmissions. The coordinator may be required to perform other related duties as assigned.
"Qualifications
Bachelor Science Nursing (BSN)
At least three (3) years clinical experience in area of specialty
Excellent communication skills, both oral and written, interpersonal, and facilitation skills
Computer proficiency including Microsoft office suite programs.
Demonstrated ability to produce results and coordinate projects.
Proven critical thinking and problem-solving skills with the ability to organize, analyze and present data
Experience with patient education
Demonstrated ability working well with patients/families, physicians, nursing and ancillary staff
Sepsis
BSN with Masters Degree in Nursing preferred
At least three (3) years clinical experience in an emergency and/or critical care setting with a solid grasp of hemodynamics, preferred.
At least two (2) years of experience with program management, preferred
Excellent communication skills, both oral and written, interpersonal, and facilitation skills
Educational process and performance improvement skills (such as Lean, Six Sigma),
Computer proficiency including: in-depth computer knowledge and the ability to navigate data bases and spreadsheets, generate reports, ability to learn and operate current patient care management systems such as EPIC and eTIME
Proven analytical and problem-solving skills
Experience with the sepsis patient population
Experience with staff and patient education
Demonstrated ability working well with patients/families, physicians, nursing and ancillary staff
License/Certification/Registration Requirements
Valid California RN License
Current BLS certifications
Ages of Patients Served
